Application For Renewal / Restoration
Please read all instructions carefully before apply :
2 passport-size and 2 I-card size photo for (I-card/name change/duplicate registration certificate/certificate change)
Original Registration Certificate.
Address proof(Telephone bill/Electric bill/Adhar card/Voting ID/Bank passbook/Passport)
Council I-card xerox
If name change occur then (Marriage Certificate/Govt. Gazette)
If duplicate registration certificate occur then (Affidavit On Rs.100 Stamp paper with magistrate stamp & police complaint FIR copy)
Send all documents Original with xerox with self attested by post to MCH office along with signature on Application form print and payment receipt.
Total fee Rs. 1600/-. If late for renewal, then total fee Rs. 4100/-
Fees paid under BHP Act 1960, section 25,26 & 27, shall not be refunded.
